The cargo vessel, Dali, which collided with Baltimore's Francis Scott Key Bridge on Tuesday, had a history of accidents and deficiencies, it has been revealed.

The ship, which remained afloat despite the collision, had previously been involved in a 2016 accident and had been flagged for deficiencies in inspections as recent as last year.

The collision resulted in significant damage to the bridge, causing it to collapse into the Patapsco River, taking with it vehicles and people. Despite ongoing search and rescue operations, only two individuals were recovered immediately after the incident. One of them is in critical condition, while the other refused medical attention.

USA Today reported that in 2016, the Dali had collided with a stone loading pier while leaving the port of Antwerp in Belgium, causing damage to its stern. The incident was attributed to an error by the master pilot.

CNN disclosed that the subsequent inspection in Antwerp identified a deficiency in the ship's structural conditions, specifically, "hull damage impairing seaworthiness."

In June 2023, during an inspection in San Antonio, Chile, another deficiency was noted in the Dali's "propulsion and auxiliary machinery — gauges, thermometers, etc."

The Dali, owned by Singaporean firm Grace Ocean Pte Ltd and managed by Synergy Marine Group, underwent its last inspection in New York City on September 9, 2023, with no issues reported.

Before the catastrophic collision on Tuesday, the ship's power reportedly failed, leading the crew to issue a mayday, stating they had "lost control." Maryland Transportation Secretary Paul Wiedefeld confirmed during a press conference that "the vessel notified MD Department of Transportation that they had lost control of the vessel and an allision with the bridge was possible."

An investigation into the incident is currently underway, and efforts to find any survivors of the bridge collapse continue. In the meantime, all commercial marine traffic at the shipping terminal has been halted, leaving several vessels stranded, unable to enter or exit the port.
